NTE5 and CAT5 wiring

Hi

I have a small phone system in my office .I have it all working over the CAT5 network as well as the DATA points .I am trying to connect a fax machine and a credit card machine over the CAT5  using my broadband line .I have connected a CAT5 cable from the internal connecotr  of NTE5 (consumer side) using the blue and blue white cables onto the corresponding cable on the patch panel .

When you plug the phone into the CAT5 socket using the adaptor lead( secondary type) it cuts out the broadband which is connected directly into the front of the same socket .When you unplug it the broadband comes on agian

Any suggestions as to where I have gone wrong would be much appreciated and alos if any one can confrim the colours/ numbers  I should have used for connecting
