I've made a couple of changes to the Rate your ISP page. 

It mostly concerns ISPs with vISPs but I'd be obliged if people could test that all is working ok and rate your ISP for this month.

Its working OK . My ISP was correctly identified and I was stopped from doing another rating as I had already done one this month .

Worked fairly well, the only problem for me is that:

Customer Support should could not be rated as I have never needed to use them, but I had to rate them to be able to complete the survey.
Would it be a idea for a "Not Used" box.

Whilst I understand where you are coming from, it would make things rather difficult for the rating process as each section gives a maximum available points which contribute to the total score.
If a particular section wasnt completed then it would skew the overall total. :/

It was something that was mulled over when deciding how best to score each ISP..  but it just made it too complicated
