course, keeping an open mind they left it on something hot but it does look like the melt patterns radiate outwards from the device as opposed to upwards ..
thoughts? user error? potential fire risk from devices?

even covered up thats nasty, seems no kind of thermal protection on the chipset.

From the date of the post then I guess it would be a HH5A, and judging by the shape of the melt, the worst damage appears to be right over the Lantiq chipset.

I wouldn't be surprised if this actually happened without him throwing it in a fire. The plastic on the HH5 is quite thin & sensitive even to sunlight. Perhaps this person had his HH5 facing a window where the glass was reflecting the sun onto it, that + the normal temperature of the board it would've been enough to melt it. IMO.
..Or perhaps I'm just an idiot.
