Does anyone know anything about the level of support for QoS in routers and BRAS devices on the internet itself?
I read some encouraging things in various docs from BT. I'm not at all sure about my ISP though, and my router doesn't speak QoS, afaik.
Second question: Check my brain, which has completely failed temporarily: Where does QoS really need to live in terms of which routers in the path? Where the bottlenecks are? Or will it help just deploying it upstream of a bottleneck, or what?

It's most effect at bottleneck, because that's where the devices need to decide which packets to forward and which to drop. Hence ISP's like A&A and Plusnet setting their profiles slightly lower than the BT Wholesale profile, so that they get to make the drop decisions and BTW can forward everything without further drops.
